Moon phases and when they rise and set and its highest point?

I have an exam over it tomorrow and I have most of it down except when exactly the phases start. The teacher told us the times are midnight 6am noon and 6pm and we have to be able to tell by those times when the moon rises sets and what its highest point is based upon if it is full, new, 1st, quarter, gibbous, and what not... i have no clue about this... also i was curious what is the 1st quarter moon... when the right side is illuminated or the left?
